Chronic Disease Management

The 14-week Chronic Disease Management registered program offers many levels of aquatic and land-based physician referred,  progressive group exercise classes. The kinesiologists and specialized trainers will help to maintain independence and functional conditioning. These programs are helpful to people with congestive heart failure, asthma, diabetes, high BP, lung disease, renal failure, depression, liver disease, dementia and other chronic conditions.

Administration Fee:

Customers that wish to withdraw or transfer from one program to another will be charged $12.25. Exceptions may be made for medical reasons only.
